All information listed in Proposal Request Form 2-50 group (PDF) in typed format.

Groups of 51-99, please provide the following:

All information listed in Proposal Request Form 51-99 group (PDF) in typed format.
A completed Census Form for all employees.
For groups of 51 - 99 employees, ODS requires current and renewal rates, or a minimum of 12 months experience, if available.
For groups of 100 or more employees, ODS requires current and renewal rates, as well as a minimum of 12 months (preferably 24 months) experience. Experience includes premium received, claims paid, and member counts broken out by month.
Benefit summaries (if any) from the current carrier.

Groups of 100+, please provide the following:

ODS target turnaround time for a proposal is as follows:
